Re: [RC] [RC] CPK - Elizabeth ChaseHopefully this is a quick question.... my understanding of CPK levels (in humans anyway) is that it may also reflect intense exertion, not only damage. The damage occurs after the lysed muscle cells release their proteins, which are very large and the kidneys have difficulty clearing them effectively if overloaded, thus causing rhabdomyelosis. Additional numbers, like GFR, creatinine clearance, ammonia, and lactic acid among others aids the diagnosis and therapy. In an otherwise healthy patient, IV fluid rescusitation is usually indicated. Occasionally someone may require dialysis. Troponin levels target cardiac damage specifically (for the lay reader, of course you know this Dr. Q). Is this similar in horses? Oh, and would you or someone please recommend a book or additional resources addressing equine metabolism? My Delphi may be insulin resistant, and we are copng with an episode of laminitis. I have the large text entitled Lameness . In horses, it is expectedly high after tying-up. In people it can be used to diagnose heart attack due to damaged muscle cells in the heart. Hope your horse recovers soon.
