I have a no longer quite new (2001) GMC long bed and a Sundown LQ 
gooseneck.  I'm kind of medium sized and I can get in between the two 
with care.  I'm more likely to hit my head on the bottom of the 
gooseneck and than anything else.  My spare and 2 propane tanks are 
mounted on the front of the gooseneck.  BTW, it is possible to remove 
the spare while still hitched too!
