Yes, injury can cause it--but it is usually the injured foot that becomes high and narrow.  The foot that is overused will wear its heels off because of landing harder and flatter on them all the time, and the lame foot will retain its bars and contract because it isn't used as much--there is less wear and less expansion of the foot, and a tendency to "spare" the heels by toeing down instead.

I have read that a hi/low can be induced by an injury to the limb of a young horse, with the "hi" foot.  The horse favors the other foot long enough that it becomes lower and flatter while the other foot becomes higher and narrower.  PP
