I did something similar to my horse several years ago, but with a screw that came through the saddle at the back. the poor horse had a lump where the screw kept irritating him. I finally took the screw out. It was holding in the crupper bar and I figured i could do without that.
 
I took my horse to the vet chiropractor. I also put some linimint and DMSO right on the lump. With chiropractic care, the horse was ready to ride pretty quickly, but it took months for the lump to disappear.

p.s btw, i retightened the screws using lock-tite but i think that this has taught me a lesson to take the time to check teh screws once a week. it seemed like a pain before but seeing my mare in that much pain convinced me that 20 minutes out of my life every week doesn't even began to compare to teh damage i could do to her.
