Hi Carol,
It's a longshot, but we used to board a TB mare who had the same
symptoms.  It turned out she had a reoccurring absess.  Both farrier and
vet worked with the mare for several months trying to pinpoint the
problem.  By the time this was diagnosed, the bones in her foot were
starting to become infected and she had to be on meds for a long time.
It might be worth it to have him x-rayed.  Good luck!
