And God bless ya for doing it. I can’t tell you the number of
times vets have to guess at what the owners might want, or how far they want to
pursue treatment, when the owners are out of contact and the caretaker doesn’t
really know for sure.
Most vets have a standard form they can provide.

We are lucky
to have a good relationship with our vet. That is he knows that we are
good for the bills. When we left last year for a month in AZ we left a
specific authorization for the caretakers, and a professional horse breeder
neighbor, to contract for care, the limitations on care, and authorization for
the vet to euthanize if in his/her opinion it was the proper course of action.
We,
obviously, left contact numbers, but knew that sometimes you can't be reached
even in our communication obsessed age.
