If they were traveling 8-10 hours at a stretch and doing it frequently, it might be an issue, but it shouldn't for shorter trips. Size of the horse makes a difference too; I know that even though there's a manger, my gelding could put his head down if he wants to back up far enough (tall trailer, 15 hand horse). I've never seen any sign of him doing that, and no one follwoing me ahs ever said he did, but he could. I've followed him with someone else pulling my trailer and he mostly looks out the window so his head would be up anyway.
