Are there any other things that might predsipose the animal to a rupture, e.g. worm damage, a server colic in the past (unrelated to an endurance ride), ulcer, or just a spot of weak tissue? Nothing is any stronger than it's weakest link. The Columbia should have brought that home in spades.
