On a tip from Lynne Glazer, I learned that the 'sore spots' are much more
visible on a wet coat.  Now, every time I hose Lakota down, I check his
withers carefully.  The sore spots he had this summer were quite visible
when his coat was wet (Lakota is bay, and the sore spots showed up as
grayish marks under wet hair), but now they are (thankfully) all gone,
though the white hairs remain.  It may be something to keep in mind, when
in doubt.
