I second gutteral pouch infection. But it could also be sinuses, I don't know if tapping is a definitive diagnosis.

Thanks, Truman. I am concerned. Especially since, a few years ago, my
older horse had his eye swell up and a discharge from the eye because of an
oral fistula, but he never smelled bad.
