I have a crew cab and can't see the ball either - here is my low cost solution. 
Take sidewalk chalk and draw a line from the ball, across the bed and 
tailgate.  Right down the middle.  Line up the gooseneck coupler with the chalk 
line and back slowly.  You will still have to get out to see when you are over 
the ball.  My truck has the spray in liner so the chalk adhere's well.  I would 
imagine you could use masking tape for a line on a painted truck bed with the 
same result.
