heres the FL site http://www.doacs.state.fl.us/ai/main/equine_ecard.shtml

It's very easy to get in FL. You send off a copy of the health 
certificate, and Coggins and digital pictures (you can take them) of 
your horse with the application and they State vets sends you back the 
card(s).

I suspect you need to contact your state vet for information.
