Seems it was not set up right or the link is missing something.  I went there and it said I had taken the survey.  Wonder if it is trying to get new members as it listed how to join??

I just went to this site, and it also told me that I had already taken the survey.  I most certainly did not. 

 

The administrators might want to know that their survey protocol is flawed.
