As long as the patient is conscious, alert, and oriented x4 (knows name, the date, place, and circumstances/purpose), s/he may refuse any treatment and/or transportation that s/he wants to. Although, you have to be careful because the medical professionals making the assessment know a few tricks to get you to goof up in answering these questions so they can claim that you weren't oriented x4 and therefore incapable of refusing treatment.
