My weight may be an advantage for my horse, but, even without pain issues, I have serious issues with caloric reserves and hydration. I think any extreme, be it a very small person or a very large person, is going to have some different issues to deal with. Even people who don't fall into the extremes frequently have some challenge or another to overcome.
