If it was my barn and I was in the situation, I would tell the woman to find somewhere else to board her horse unless she gets a vet out to see him.  At least you don't have to deal with her, and that negativity doesn't have to be in your barn.  If she wants to be a poop about her horse, let her do it somewhere else.

It's a similar situation that vets have to deal with.  Someone comes to you and wants you to do something that is not morally right.  You know if you say "no" they'll just take the animal somewhere else and have it done.  It sucks, but it sounds like this woman is either not aware of how serious her horse's problem is, or doesn't care.

You could also see about your local regualations for animal welfare, and make a report to them.  Maybe having an officer out there will clue her in to how serious it is.  Worst case, she'll take her horse somewhere else, but maybe she'll see the light.
