The problem is how to get people to volunteer to die at a young age? 
Particularly since many if not most of the people who used to die young were 
below the age of consent.  Or perhaps you mean that women should not have 
any medical help so that the proper number die in child birth?

While Alzheimer's is certainly a long and bad way to die, I don't think that 
disease like smallpox, malaria, diphtheria etc. are pleasant either. 
Certainly, death by appendicitis, or septicemia are also nasty.
