Our Trail Club just enacted this for the same reason...no one knew who the person was, what rig was his, etc.  Our forms have all the person's information including who to call in an emergency, if they are allergic to anything and where their meds are (bee stings being one case), the description of the rig and truck license number.  Now we know who has what, who to call, and especially who's there with us, etc.
 
Now, I go one step further and have always carried a lamenated card with me.  It has all my emergency contact info on it, rig description, where the horse is boarded (at home now), that I'm allergic to bees and my kit is on me, etc.  This way if anything happens to me, even if I'm with one of my riding buddies they'll know what to do.  For gifts one year, I made cards for all my riding buddies with their info on it so they had them....especially the ones who ride alone.

I was working the Tour De Washoo ride a few weeks back. A lady got bucked off her house and hurt. The horse was OK and Tom Sherwood rode it down the mountain back to camp.
Here is where it got a little sticky. The lady had come alone- like MANY of us do- and no one knew the exact description of her truck/trailer. After awhile, the correct rig was found, and the horse was taken care of properly, and volunteer found to drive the horse back to correct home.
My suggestion is this:

Is there room on ride entries for some more spaces of info: Make/color/license plate of truck and same for trailer? If this info had been on the ride entry, less confusion would have occurred and if in the case of the horse requiring some attention other than hay and water, at least ride management and volunteers would have been expedited in their efforts.
Can anyone who manages a ride tell me if this is just extraneous garbage or something AERC should consider adding to ride info? Or up to individual ride mgrs?
I am still driving my 1976 Chevy dually which alot of riders recognise, but I have a newer 4 horse trailer which alot of them don't recognise. Realizing that quality volunteers would be attempting to properly care for my horse if I am hurt, I would like to have their efforts not thwarted or delayed by lack of information.
