I carry this info. with me when ever I ride.  I made up a little card with my name, DOB, blood type, horse info., vet. phone #, emergency contact numbers, veh. desc. and license plate #.  Then I laminated it.  I keep it in my pocket (and one inside the door of my trailer).  I figured if I came off and was hurt so badly that I couldn't speak, then rescue personnel would go through my pockets and find it.  And if I'm still hurt pretty bad but can communicate, I can direct rescuers to the info. on the card.
