Hi Lisa,
My gooseneck sits on a ball that's bolted to a plate in the bed. I 
needed 2-3 more inches of height and found a factory hitch ball made 
with an extended shaft so I just unbolted the old and on with the 
new. Bolt on 30K 2-3" Highrise Extension here

I'm sure a good machine shop can extend them to the 4" that you need.
