Yep, we put all that stuff down on the forms.  It would be a data entry
thing, I'd imagine--since the name is entered, it just needs the
additional spots there to add gender and year of birth.  (Color might be
nice to add as well, just for reference.  "Is this her bay horse or her
grey horse??")
