This requirement comes and goes with VS.  When VS is hot, surrounding states
get pretty sticky, and when it gets more under control, the requirement goes
away again.  The Canadian border was even requiring inspections by their
border vets for temporary permits for awhile, in addition to having to have
the "pat statement" on the forms.

The BEST advice is to always insist that your vet call the state in question
for up-to-the-minute regulations, because they DO change with disease
outbreaks, etc.
