I say the results of a saddle slipping with at least a breast collar
on.  (I can't remember if there was a crupper.)  The rider had leaned
over for a gate, and the saddle slipped over and the rider came off.
The horse took off for about mile.  When caught, it had the ugliest
looking burn in the armpit from the twisted breast collar rubbing.
