Yup. Look carefully to be sure it's not twisted, stirrups are
symmetrical, etc.

Not necessarily. Doesn't sound like a pad-fixable thing to me. I'd be
more inclined to look at the girth, the rider, the saddle, and an injury
in that order.
Hope it turns out to be easy (& cheap) to fix.
