Need sample lease

I am getting ready to lease one of my  mare to a person in another state for
breeding purposes. What things should be included in the lease arrangement and
are there any samples available?
I have had this mare for 15yrs and while I trust the person to whom the horse
will go, I want to protect myself and my horse.  Anybody have any comments?
