I too have a bad knee (degenerative cartilage disease) and have found 
that in general, longer stirrups and posting at least a minute or two 
every 10-15 minutes helps a lot.  And in testing out new saddles 
recently, I learned that a narrow twist on the saddle is better for your 
knee, probably just as the narrower horse is (as Angie said).
