Gnats in ears

Every spring my horses start rubbing and digging at their ears. There is often a swollen area below the ear. I use Gnat-Away and try to keep their ears covered with those mesh protecters. Is there anything else to do? Its got to be very uncomfortable for them. I'm in the southwest.   Julie
