I got mine at the local grocery story (it's an all purpose store) they cost $5.00  It's a machine in which you insert $5 and type the information on the screen and push print, total time about 5 minutes.  There are several shapes depending on how much information you want on it.  I have my horse's name, owner (my name), city, state, and phone.  I live in Oregon, but would be happy to get them for you and send them to you if you don't have an place to get them there.
