My friend's horse tied up bad on a training ride.  He couldn't move a
 rear toe at all...but his pulse was 32 and he ate everything he could
 reach.  That's why I think an exit CRI is important at this level. Some
 of these horses could reach criteria with a leg missing. :-P
I had another friend who leased a horse for the World Championships in
 Kansas.  The horse had done something like 18 100 milers, 4000 miles of
 endurance...never been treated, no metabolic problems.  Was known as a
 "mule" (not really) who wouldn't allow himself to be over ridden.  She
 crewed for the leaser. The person who leased him followed her
 directions perfectly and she thought he was a very thoughtful rider...
 the horse completed, then crashed and almost died. He spent several
 weeks in an animal hospital...had to have a part of his jugular removed
 after aquiring a staff infection...not fun!  Whose fault?  Partly mine
 I guess since I told her leasing him was a good idea.  No ticking time bomb that I now of in him.  He went on to compete again for his owner.  I think everyone there did their job, vets, rider, crew, horse...but the rider didn't *know* that horse.
  
I read an account of the press conference after the WEG in which the
 press asked many questions about the horse deaths and some (I think
 participants) critisized the fact that the horse was leased and the
 rider didn't know him.  Then, I read the ride write up and see that the
 winner was riding his horse for the first time.
What does all this mean? I don't know. Just more anecdotes to weigh
 before forming an opinion.
